This short film focuses on a GP surgery and the Adult Services they provide in Barking and Dagenham.

 

The film takes us through how a GP and local authority initiative created six teams based in GP practices and how they have integrated primary healthcare and social services. 

 

We see how this restructuring has enabled people to work together, to deliver successful integrated care in Barking and Dagenham.
